Enrollment Procedures for Junior High School

You can pre-register to our school by clicking the Google Forms button. Fill out all the necessary information and ready the following requirements.

• Photocopy of PSA Birth Certificate
• Grade 6 Report Card (Original)

• ESC Application Form provided by their Advisers
• Recent Identical 2x2 I.D. Photo with white background (2 Copies)
• Photocopy of PSA Birth Certificate
• Grade 6 Report Card (Original)
• Barangay Certificate
• Student's Full Name
• Parent's Name and Current Employment
• Monthly Income
