Enrollment Procedures for Educational Units / Graduate Studies

You can pre-register to our school by clicking the Google Forms button. Fill out all the necessary information and ready the following requirements.

• Honorable Dismissal
• Transcript of Records or Certification of Grades for Evaluation (Original)
• Barangay Clearance
• Photocopy of PSA Birth Certificate
• 2x2 I.D. Photo with white background (2 Copies)
• Marriage Certificate (PSA) (If married female)
• Transcript of Records with Remarks (Original) Copy for Imus Institute of Science and Technology, Inc. (Required after officially enrolled)
