Enrollment Procedures for Senior High School

You can pre-register to our school by clicking the Google Forms button. Fill out all the necessary information and ready the following requirements.

• PSA Birth Certificate (Original)
• Report Card (Original)
• Certificate of Good Moral Character
• 2x2 I.D. Photo with white background (2 Copies)
**ESC/QVA Certificate (for SHS Voucher Recipient (from Private School))
