Enrollment Procedures for College
You can pre-register to our school by clicking the Google Forms button. Fill out all the necessary information and ready the following requirements.
Requirements for College (Freshmen):- Photocopy of PSA Birth Certificate
- Senior High School Report Card (Form 138)
- Certificate of Good Moral Character
- 2x2 I.D. Photo with white background (2 Copies)
- Form 137 (Required after officially enrolled)
- Honorable Dismissal
- Transcript of Records or Certification of Grades for Evaluation
- Certificate of Good Moral Character
- 2x2 I.D. Photo with white background (2 Copies)
- Transcript of Records with Remarks (Original) Copy for Imus Institute of Science and Technology, Inc. (Required after officially enrolled)
